<p>I enjoyed parts of <a href="https://openlibrary.org/books/OL29374439M/Inflamed_Mind">Inflamed Mind: A Radical New Approach to Depression by Edward Bullmore</a>. It is very well written and has some good parts on the history of psychiatry.</p>
<p>However, I was disappointed by the way he presented his main thesis that inflammation is the <em>main</em> causative mechanism in major depression. His argument isn't very convincing and he doesn't provide evidence that is solid enough to support this. There is also a major straw man throughout the book. Bullmore claims that psychiatry is stuck in Cartesian dualism yet he doesn't discuss the biopsychosocial model and rejects any psychological explanation of depression as stigmatizing. The modern scientific way of thinking about the relationship between the mind and the brain is that <a href="https://ajp.psychiatryonline.org/doi/full/10.1176/ajp.155.4.457">all mental operations arise from functions of the brain, including higher functions</a>.</p>
<p>Fortunately, he is careful not to offer false hope or push unlicensed or unsafe treatments on vulnerable patients.</p>

<p>Tags are non-hierarchical way of organising your data. The keywords you use to describe your data are on the same level. They became very popular with social bookmarking around 2004. Many systems offer tagging as an option when editing the metadata of your files.</p>
<p>Tags offer flexibility in classifying your data. You don't have to worry too much about where your data fits in relation to others. However, this flexibility can lead to overuse or confusion.</p>
<p>A common example of problems facing tags are the issues of homonyms (the orange tag may be meant for the color or the fruit) and synonyms (different tags for the same idea).</p>
<p><a href="https://karl-voit.at/2022/01/29/How-to-Use-Tags/">Karl Voigt has an excellent post on tags and how to use them</a>, in summary:</p>
<ul>
<li>Use as few tags as possible that are self-defined</li>
<li>Decide on singular or plural</li>
<li>Lower case</li>
<li>Omit tags that are obvious</li>
</ul>
<p>Tags can also be displayed in a <a href="https://moftasa.net/bookmarks/tags">tag cloud</a>.</p>
<p>Although multiple operating systems now let you tag your files. Tags are not very portable. They may not be copied over to a different system or when sharing a file by email or through a network.</p>

<p>Naming files in a consistent and logical way can help you stay organised. You should:</p>
<ul>
<li>avoid relying on your memory,</li>
<li>be able to preserve your ability to recognise your files years from now.</li>
</ul>
<p>First I would like to introduce you to the ISO date format. This is a way of writing the date in a way that makes computers sort your files from oldest to newest automatically. <a href="https://www.bruh.ltd/blog/iso-8601-the-better-date-format/">This blog post explains it in more detail.</a></p>
<p>I personally try to stick with the following system:</p>
<ul>
<li><code>YYYY-MM-DD_TYPE_description-of-content_AUTHOR_version.ext</code><ul>
<li><strong>YYYY-MM-DD</strong>: The date in ISO format.</li>
<li><strong>TYPE</strong>: This is the type of file from your point of view not the point of view of the computer. From the computer point of a view PDF file is a type of file but for me it may contain a presentation, research article or a legal document.</li>
<li><strong><dfn title="Joining-text-with-dashes-is-called-a-slug 🐌 and it is optional but is good practice as computers didn't handle spaces in filenames well in the past.">Description-of-content</dfn></strong>: This is the main description of the contents of that file.</li>
<li><strong>AUTHOR</strong>: Author initials. I only use this if the file is part of a project with other collaborators.</li>
<li><strong>version</strong>: To keep track of changes or revert to earlier edits.</li>
<li><strong>.ext</strong>: the file type extension such .txt, .pdf, etc..</li>
</ul>

<details><summary><strong>Final tip:</strong> Try to left pad numbers in filenames with zeros.</summary>

Both books recount their authors' lived experiences with quite severe and acute neurological conditions. Taylor had a haemorrhagic stroke and Cahalan had <a href="https://en.wikipedia.org/wiki/Anti-NMDA_receptor_encephalitis">anti-NMDA receptor encephalitis</a>.</p>
<p>I picked up Taylor's book while supporting a family member in hospital. I must say I really enjoyed reading her account of what happened on the day of the stroke and how she managed to get help. The book focuses on her recovery, which took 8 years. I found her concept of <em>tending to the garden of one's mind</em> and her list of things she needed most during her recovery very informative.</p>
<p>Cahalan's book was a much more exciting read. Anti-NMDA receptor encephalitis is a rare condition that presents with a range of severe psychiatric and neurological symptoms. Reading about how it manifests and how the diagnosis was made was very exciting.</p>

<details><summary>😳 The way people organise their files can be very surprising.</summary>
<p>In a place where I used to work, there was a colleague who was responsible for writing and handing out medical certificates to patients. These certificates are a one-page document containing basic information and the length of treatment.

She saved all the certificates issued since she started working in a single .doc Microsoft Word file. One certificate after another from different patients, separated by page breaks. It was clearly a system that worked for her in the beginning, as it was easy to search by patient name or date. She didn't have to worry about file names or folder structure.

<p>
Over the years, this file grew huge and Word struggled to open a document containing more than 10 (or maybe even 20) years of data. Imagine if that file got corrupted! I tried to convince her that it would make more sense to have a single file for each patient, but she didn't like my suggestion, but agreed that a single large file was too much for the computer's RAM. She agreed to create a different file for each year.</p></details>
<h2>Folder Hierarchy</h2>
<p>It pays to spend some time thinking about how you organise your files on your computer. It can help you find things quickly. Your computer's file system can be a very neat database or a disorganised data dump. Once you decide on rules for how your files are named, stored and relate to each other, this common tool becomes a very useful database.</p>
<p>Think about your downloads folder. A messy collection of random files. As it fills up, it can become difficult to find what you want. Imagine you have a special folder for ebooks, with subfolders for major subjects. A folder for <em>Fiction</em>, <em>Cooking</em>, <em>Psychology</em>, etc... and you have been careful to categorise the ebooks correctly. As you download more books and store them properly, your database will grow and your ability to find the ebooks you want will remain effortless.</p>
<details><summary>⛔ You may quickly run into the limitations of this system, but that's OK.</summary>There are solutions. Take for instance a book called "The Psychology of Cooking." In this case, you could save two copies or categorize it as a psychology book rather than a cooking book, considering it lacks recipes. Alternatively, you could use the shortcut feature in Windows (symbolic links in Linux) to access it from both Cooking and Psychology folders.</details>
<details><summary>🌈 Imagine how a fictional clinical psychologist might organise his or her work documents.

This is certainly better than dumping everything into My Documents. This may not work for everyone, and you may want to organise the folder hierarchy differently. That's fine, but once you do it, you have to stick to the system and respect it for it to pay off later. You can change it later, but it shouldn't happen too often.

<p>There are recommended systems for naming your folders, such as the <a href="https://johnnydecimal.com/">Johnny Decimal system</a>, and standardised ways of classifying certain things, such as the <a href="https://www.bisg.org/complete-bisac-subject-headings-list">BISAC subject headings</a> for books that can act as starting templates.</p>

<p>Losing data can be frustrating. I've learnt the hard way that syncing data to another computer or to a cloud provider (e.g. Dropbox) is not a good way of backing up data. Corruption in a single file can spread to synced copies<sup id="fnref:vc"><a class="footnote-ref" href="#fn:vc">1</a></sup>.</p>
<h2>3-2-1 Backup Strategy</h2>
<p>My search for backup software led me to the recommended 3-2-1 backup strategy, which effectively minimizes data loss risks:</p>
<ul>
<li>You should have 3 copies of your data. One original and two backups.</li>
<li>Store the data in 2 different locations.</li>
<li>1 copy should be kept in an off site location.</li>
</ul>
<p>For me this means I keep a dedicated external hard drive connected to my desktop computer for daily backups. Modern software<sup id="fnref:recommendation"><a class="footnote-ref" href="#fn:recommendation">2</a></sup> can encrypt and automate this process.</p>
<p>Every six months, I update a second external hard drive with a fresh encrypted copy of my data. This second drive is kept with someone I trust for safekeeping.</p>

It used to be a single <a href="https://tiddlywiki.com">TiddlyWiki</a> HTML file that displayed all my bookmarks (3900+) which was easy for me to setup but didn't make sense that each visitor would be served a 5MB file just to browse my bookmarks.</p>
<p>I still use TiddlyWiki to store my bookmarks on my devices but I managed to export a CSV of all the bookmarks and put together a …</p><p>I have updated the <a href="https://moftasa.net/bookmarks">bookmarks section of this website</a>. It used to be a single <a href="https://tiddlywiki.com">TiddlyWiki</a> HTML file that displayed all my bookmarks (3900+) which was easy for me to setup but didn't make sense that each visitor would be served a 5MB file just to browse my bookmarks.</p>
<p>I still use TiddlyWiki to store my bookmarks on my devices but I managed to export a CSV of all the bookmarks and put together a python script that converts each row into a markdown file. I then used <a href="https://getpelican.com/">Pelican</a>, which is the same static site generator that I use for this blog, to create the static version to be served to the public here.</p>
<p>Pelican is pretty accessible if you have basic Python and HTML skills. I modified the theme I am using here, which allowed me to add a <a href="https://moftasa.net/bookmarks/tags">tag cloud</a> and I am particularly proud of being able to automatically add an optional link to the internet archive for old links.</p>

<p>I was in the audience of a panel titled <strong>Data Governance for AI in the Health Sector</strong> which was more about data unavailability than AI. <p>I was in the audience of a panel titled <strong>Data Governance for AI in the Health Sector</strong> which was more about data unavailability than AI. Prof. Fatima Roumate raised an important point about the sovereignty of data and systems, Meriem Ben Tarjem addressed the emigration of doctors as being one of the most pressing issues in Tunisian healthcare while some of the very important concerns were raised briefly at the end by <a href="https://shamseya.org/">Dr. Ayman Sabae</a> on possible harm of AI, its inevitable use in health insurance which may threaten access to care.</p>
<p>As for actual implementations of AI in the region, I learned about <a href="https://rology.health">Rology</a> which is a teleradiology startup that uses a combination of AI and radiologists in their reports.</p>
<p>I would also worry about big tech monopolies in this sector as local companies my not be able to compete and the real threat that LLMs pose to patients in regards to <a href="https://pubmed.ncbi.nlm.nih.gov/37881016/">misinformation</a>, <a href="https://doi.org/10.1038/s41746-023-00939-z">propagation of bias</a> and its inability to communicate uncertainty.</p>

<p>Try to set <a href="https://techwiser.com/how-to-automatically-delete-whatsapp-messages/">Whatsapp</a> or <a href="https://www.howtogeek.com/747458/how-to-make-all-your-signal-messages-vanish-by-default/">Signal to automatically delete messages in your chats after 90 days</a>. This automatically reduces the patient information that may be on your phone.</p>
<p>For video calls, Zoom became the default during the pandemic. Their privacy and security practices <a href="https://www.theverge.com/2020/4/1/21202584/zoom-security-privacy-issues-video-conferencing-software-coronavirus-demand-response">haven't always been the best</a>. They claim that they offer E2EE, but this is not the case if a user uses the browser instead of the official app.</p>
<p><a href="https://meet.jit.si/">Jitsi</a> is the FOSS<sup id="fnref:2"><a class="footnote-ref" href="#fn:2">2</a></sup> alternative to Zoom. Initially, it couldn't compete in terms of performance to Zoom but now it has improved considerably. <a href="https://jitsi.org/e2ee-whitepaper/e2ee-pitfalls/">Despite offering E2E encryption they state that there may be limitations</a>.</p>

It can be a good idea to add another factor of security. Some password managers lets you use a Key file (can be stored on a removable USB thumbdrive) or a hardware key (such as YubiKey).</li>

Knowing that their data is secure, patients feel comfortable disclosing personal details crucial for accurate diagnosis and treatment.</p>
<p>According to where you work you are required to follow certain laws regarding data protection. In Egypt, things are limited. We depend on paper records and IT departments aren't as resourceful. Occasionally you have to write a medical report on your laptop. Sometimes, patients contact you via WhatsApp, where they share a lot of information or forward their lab results. A stolen phone or a hacked laptop can compromise their safety.</p>
<p>In the next posts I will share tips I learnt over the years from different sources. However, computer security is a serious discipline, and how vulnerable your machine is depends on many variables. I am not a qualified security expert. So please consider this as simple computer hygiene.</p>

<h2>Reflective Practice (RP) 🪞</h2>
<p>We have been running monthly RP groups at our hospital and it has been very popular. I learned about it from an RCPsych newletter and thought that this was a good idea to get trainees to share, reflect and learn from their encounters.</p>
<p>The value of RP is that it lets you stop and think about what you are doing, this helps your learning. Since this is only a quick post on the topic, I would like to highlight the following points:</p>
<ul>
<li>RP can be done individually on your own or in group.</li>
<li>Write things down, this will make it a much more valuable exercise.</li>
<li>This is a <a href="https://drive.google.com/file/d/1WA36yCfV3dW2VpdVU6dsgE5Xqfm_ceRG/view">simplified form that you can print</a> and use in groups or individually.</li>
<li>RP isn't only for negative or difficult clinical encounters.</li>
<li>Revisiting your notes is a great way to evaluate your growth.</li>
<li>RP isn't exclusive to psychiatry.</li>
</ul>

<li>🎙️ <strong>Ask yourself if interviewing the patient during the case presentation is necessary</strong> for the diagnosis or management of the case. This minimises patient's distress and shows that you value their privacy.</li>
<li>🏥 <strong>Present more outpatient cases</strong> to enhance your learning experience.</li>

<p>أنا طبيب نفسي وأشتغلت في مستشفيات نفسية وجسدية ف أعرف كتير عن أزاي المستشفيات بتشتغل، كمان أتكتب كتير عن التجربة المعكوسة لما يكون الطبيب هو المريض أو قريب المريض، لكن لأني تحديت نفسي بالكتابة أكثر هنا، عندي كلمتين عاوز أقولهم.</p>

<p>الأنتظار عذاب والإجراءات الإدارية أثناء الدخول والخروج عذاب والمرض نفسه عذاب والتفكير في المستقبل عذاب.</p>
<p>في المستشفيات النفسية الإجراءات بتاخد وقت والانتظار مؤلم للمريض وأهلة، لكن فيه بعض الأهتمام بالمريض واحتياجاته أثناء الدخول أكتر من المستشفيات الجسدية، لكن ده طبعاً نابع من القلق تجاه أن المريض في الأغلب داخل لأنه خطر على نفسه أو الآخرين.</p>
