In 2005, the International Society of Addiction Medicine established a process for physicians to demonstrate expertise in the treatment of addictive disorders. This process of international certification entails demonstration of structured clinical experience in addiction treatment and successful completion of an examination testing candidates’ clinical expertise.
Is it possible to ethically treat someone with post-traumatic stress disorder (PTSD) developed as a result of inflicting torture on others? That's the question posed in a paper published last summer in AJOB Neuroscience, but there's a twist. The paper is written and illustrated as a comic book by Lehigh University artist and neuroscientist Ann E. Fink.

The orexin system in the lateral hypothalamus and dorsal medical hypothalamus/perifornical area is activated during wakening and silent during sleep. It is the system that is disrupted in narcolepsy. It is also the system that coordinates the activity of the other arousal centers in the brain including the TMN-HA, LC-NE, DRN-5-HT, VTA-DA, and cholinergic neurons in the Nucleus Basalis of Meynert (NBM-Ach).

Death of patients by suicide can have powerful effects on psychiatrists. We report the findings of a survey completed by 174 psychiatrists on the effects of patient suicide on their emotional well-being and clinical practice, and the support and resources they felt would be helpful.
!!!Results and clinical implications
The death of a patient by suicide usually had a major effect on respondents. Clinical practice was often negatively affected, and over a quarter of respondents considered a change of career path as a result. There were some gender differences in responses, with women reporting more sense of responsibility for the deaths and a greater effect on their clinical confidence. Torture remains an ongoing global problem. Many individuals who have migrated from areas of conflict and political instability have suffered some form of torture. Survivors of torture can present significant challenges to general practitioners, voluntary bodies and mental health services. This article describes the many physical, psychological and social effects of torture. An understanding of these will hopefully aid comprehensive assessment of survivors of torture by local mental health teams. Also described are the current treatment options and their evidence base.
